Your First Visit
From the moment you arrive, our goal is to help you feel comfortable, heard, and cared for. Our providers take the time to listen, answer questions, and work with you to create a care plan that fits your life.
What to Bring
Here’s a quick checklist to make your first visit smooth:
A photo ID
Your insurance card
A list of any current medications
Any recent medical records (if you have them)
Completed forms (you can fill them out ahead of time or at the office)
Tip: Arriving about 15 minutes early helps us stay on schedule and gives you time to get settled.
